Changed yes/no in feature request template to checkboxes #5

Under the question "If the feature request is approved, would you be willing to submit a PR?" I changed the response template to checkboxes. This matches the template for PRs (under "Does this introduce a breaking change?")

What is the current behavior?

Unclear what one is supposed to do when responding to the Yes/No question -- bold the answer? Delete the non-answer? And the italicized help text makes it a little more confusing.
