Add focalplane model support for stuck / broken position

When marking a positioner as broken / stuck in a focalplane state log, we should add support for specifying the position at which it is stuck. We could do this by adding additional columns to the state log ECSV file. This can be done in a backwards compatible way. We should add corresponding options to the desi_update_focalplane_log script.
