Add githooks and actions for enforcing workflow:

Build on PR to master
Githooks
- Prevent commit to master
  - if not, rm .gitfetch on push to master (to watch for doc update)
- see Githook improvements template.hapi.rest#9
  - enforce branch naming conventions?
