SVG documents must not have DOCTYPE declarations

Per the EPUB 3.0.1 specification, content documents, including SVG documents, may not have external identifiers in their DOCTYPE declarations, if present. Since this would result in a doctype declaration like

<!DOCTYPE svg >

Which is pointless, it makes more sense to simply omit the DOCTYPE declaration completely.

In the case of SVG documents this means that documents must be processed to remove the declarations.
