Add Patreon as a way to support the project

I used Flattr in the past, but they didn't scale the way Patreon did, so these days there are a lot more people (me included) who have active Patreon accounts (as opposed to Flattr, Gittip/Gratipay, BountySource/Salt, etc) and would have a convenient way to support the project if it were to create a page there.

In the meantime, accept my sincere thanks for building this tool, open sourcing it and providing it to all for free — you guys rock!

This is a personal fork of the project and wasn't updated for quite a while. I recommend posting suggestions to official project forums.
