Using builtin Brave browser
Multi-tabbed app with Gmail as one of the tabs
Latest version of Epichrome and app is newly created

The behavior I'm hoping for is the PDF attachment opens in my app only. But the PDF is opening both in the app and the default browser. I've tried adding rules to Epichrome but I can't seem to influence the behavior.

Are these PDF attachments that appear as a thumbnail at the bottom of Gmail messages? When you say you want to open them in the app, do you mean bring up the preview in the Gmail window, or opening them some other way? Epichrome Helper only handles actual web links (<a> tags, technically), so I'm not sure how it's happening that PDFs are being sent to your default browser. The more specifica you can be in what you're clicking and what's happening, the better I'll be able to recreate it and figure out what's going on.

Can you also send me a screenshot of your Epichrome Helper options page so I can see what rules you have?

Meantime, have you tried checking the box at the bottom labeled Stop further handling for links sent to default browser (check this if links sent to default browser are erroneously also opening in this app)? I don't think that will help your situation, but it can't hurt to try.
