[Swag Infra] Create a e-form/ or use Survey Monkey to collect information from MicroProfilers

Secured/Private method way to collect information from the MicroProfile Community.

MP pays for SurveyMonkey, 1 seat currently Edwin.
What is your take about using that seat to create an "e-form" where we collect Swag information from Committers & Community?

e-form

Either we use Survey Monkey (my preference) for it as it already has the privacy included into itself with MPWG SurveyMonkey Data Protection Impact Assessment (DPIA) or we create an independent e-form using the private microprofile-swag@googlegroups.com.

The results from the data received will be added to the Private Spreadsheets that contain personal MicroProfilers information.

E-form minimal requirements

1. Name
2. address
3. phone
4. email
5. Shirt size
6. Q: Do you present/speak about MicroProfile Technology at conferences? Yes or No
7. Only if yes triggers on Q6, Do you wish to participate in giveaway MP shirts during your in-person events? Yes or No
8. Do you want MP stickers? Yes or No

I see your feedback, @Emily-Jiang. There is a discrepancy where IBM could be mentioned as it is for Fujitsu.
I think we ought to only mention the Implementors and not the companies. Payara is a special case b/c its name is used in all versions available.

What are the thoughts from others on this?
