Hyperlink Open Ethnographer codes in the Annotator.js popover

Our ethnographers using Open Ethnographer would like a way to edit code description fields more comfortably when working in the coding interface (reference: this discussion).

We settled on the following proposal: When viewing an existing, fully saved annotation, the Annotator.js popover should link the code to the corresponding page of the code, as found in the Open Ethnographer codes administration interface. To illustrate, the thing under the mouse pointer would be hyperlinked:

Because this issue is specific to the JS library used to create annotations, implementing this should wait until we have switched out Annotator.js with something that is still maintained.
