[Kibana/Management] Remove disabled "kibana" breadcrumb

In order to meet the standards laid out in #25689 we should change the last breadcrumb for the Index Patterns, Saved Objects, and Advanced Settings pages to reflect those pages rather than just say "Kibana". Sub pages within these sections should also update the breadcrumbs, like the "Edit Saved Search" page which should be migrated to the breadcrumbs: Management / Saved Objects / Edit
