Implement other boundary conditions for Geometric Flow

@sobolevnrm, only "mdh" is implemented, adding "zero" would be easy to add. What other boundary conditions do you want to see implemented?
Should use APBS's boundary condition code for this instead of implementing our own from scratch.

"mdh" is sufficient; "sdh" would be good, too. The ultimate goal should be
"focus".
