When a user clicks on the sound indicator icon in the wingpanel and then changes the volume level with another click
on the volume slider, the beep sound confirmation uses the previous volume sound level and not the "current" volume.
This can lead to a bad user experience when, for example, the user has the volume at max,
puts headphones on, then clicks on a low volume and get ears blasted with a high volume beep sound.
Cannot confirm it happens outside my install, I have both PulseAudio 11.1 and ALSA k5.4.0-72-generic,
not sure if those affect this issue or not. Wingpanel version: 2.3.2+r454+pkg48~ubuntu5.1.5.1

I guess you are running the current elementary OS 5.1 Hera stable release?
I wanted to look at fixing this issue but I'm unable to repro this issue on my 6.0 Odin early access installation and I'm not sure if the sound indicator is getting further updates on 5.1 Hera at this time.
