Splitting this file results in:

 ./src/pagestream/cli.py contents ~/Downloads/wob/Documenten\ bundel.pdf
Page 166
- 251 pages

That's because the outlineitems after page 166 don't contain valid destinations.

We could flatten the tree to solve this issue.
