Apologies on being just a little tacky with a drive-by fix, but I edited the readme directly to update it to: http://glslify.github.io/glsl-combine-chamfer/

I'm not aware there's a github pages site with a CNAME, so that I think http://glslify.github.io/ is the appropriate base URL for modules that have been moved from stackgl into glslify. Note to self that this should really be a patch release so that npm picks up the change as well 😬
