Consolidate code in a single file

Since Simulacra.js is so short and it's a design goal not to exceed a certain degree of complexity, it may be simpler to put it all in a single file. This has the added benefit of removing the build tool.

Hmm, maybe this is a non-issue, since the Browserify boilerplate is not that heavy, and I guess most people will just use their own build tool instead of downloading the minified version.
