If something else sets MANPATH earlier.

Perhaps manpath --quiet should be used instead?

tom@whaam ~ $ perlbrew --version
/opt/perlbrew/bin/perlbrew  - App::perlbrew/0.46
tom@whaam ~ $ manpath --version
manpath 2.6.0.2
tom@whaam ~ $ man manpath
[....]
DESCRIPTION
   If $MANPATH is set, manpath will simply display its contents and issue a warning. If not,
   manpath will determine a suitable manual page hierarchy search path and display the results.

   The colon-delimited path is determined using information gained from the man-db configuration
   file - (/etc/manpath.config) and the user's environment.

Sigh. The fix for this has been broken and refixed a couple of times now. Once again, it's broken, this time by daa2c94. I provided a fix as #427.
