Exception cleanup (#250)

* Make Error exclusively for authors.
* Fix syntax for throwing and creating DOMExceptions.
* Fix examples.
* Fix references to simple exceptions in legacy DOMExceptions.
* Stop exporting ES throw.

Closes #247.
