We should support the differences between gender identity and (administrative) sex.
We cannot extend the FHIR ValueSet for gender, but we can

• use a standard extension for GenderIdentity
• define our own valueset if one is needed / appropriate
• define what to do with "other" gender identities

WG: we need an administrative gender which reflects the gender of the national number. We need a gender that represents the gender at birth, and we need to have a "social" gender in the logical model. None of the genders are mandatory.
