Move vaccine allergies to CH AllergyIntolerance exchange format

Move the vaccine allergies to the CH AllergyIntolerance exchange format so that the allergies are not scattered and defined in several exchange formats and are all combined in one exchange format.

The allergyIntollerance is base on the IG allergyintollerance, but the valueset is reduced for ch-vacd to have only the selection of vaccination relavant allergies.
This is also relevant for CDS Systems to have valuesets reduced to the topic.
it could only be possible, if the ch-allergyintollerance is separating the values in different valuesets and binding together with a master valueset as decribed/requested in issue 86
