Currently, labels won't appear on the forms if you have a custom component with a config flow that doesn't use the same directory name as its domain. This is due to helpers/translation.py checking to see if the names don't match and then returning silently without logging.
This was difficult to hunt down without going to the source code and seems like unexpected behavior.
