Add data element type

The dicom parser and writer use a special storage for dicom data elements in the form of a object with key the dicom key (8 digit formed from the group and element) and property the data element. At its minimum, the data element is just a VR (value representation) and a value. This must be reflected in the internal types and API of the library.
