error using zabbix_mssql_version.vbs script

after adding script zabbix_mssql_version.vbs to my config file
MSSQL Memory manager application working like a charm, but MSSQL Server application items return:
Missing parameters
C:\Program Files (x86)\Zabbix Agent\scripts\zabbix_mssql_version.vbs(201, 12) Microsoft VBScript runtime error: Subscript out of range

server: windows 2008r2
sql server version: 2012

can you help me to resolve the problem?
