Update for ACF 5.4.5?

This is exactly what I need. Unfortunately it doesn't seem to be working with ACF 5.4.5. Specifically the Pages by template type is not showing up as an option in ACF.

Any plans to update?

If I could find the time then yeah I'd love to update it. Perhaps I'll do it today if I find myself with nothing to do..

You're also very welcome to fork it and send a pull request if you do it yourself :)
