While I've been testing in the simulator, I often forget to switch from the home network location on my mbp. So with the simulator also having no net access, I noticed that no ad is served.

As you'd expect.

From the point of view, encouraging users to buy my app, from annoyance of ads, having nothing looks odd.

I know this isn't a common issue and obviously saving an ad is pointless as can't redirect.

But a placeholder of some kind maybe with some default text, would be a great feature.

This is an interesting feature. I had been toying with the idea of including something like a small 'x' in the corner over an ad, and when it is tapped, it will initiate a custom action (such as prompting to purchase an in-app purchase to remove the ads). This takes that one step further by adding a 'house' banner when no other ad can be shown.

Ideally, this would be some kind of png or image that you could specify to display when no other ad networks are serving ads.

Closing for now since showing no ad is the expected behavior. I'd be happy to accept a pull request for a new ad adapter that displays a placeholder image.
