Do you know if case-insensitivity is explicitly mentioned in some Shapefile format documentation, and not just implied by its common use in Windows-land? I can't seem to find any info about this.

Fails to load in tilemill due to the different case of the dbf file:
QGIS is able to open so I guess we might want to adapt to this, but I could go either way...
