Cancelling image dialog creates a placeholder #3

Steps to reproduce

Press Ctrl+G for insert image dialog
Press Esc or cancel the dialog without changing input

Current behavior

Creates an image placeholder, like this:

This happens even if the input is emptied.

Expected behavior

Do not add anything when cancelled.
