Validation doesn't report character offsets

What happened?

markdoc.validate reports start/end lines but no character offsets. This becomes a problem when there's multiple different issues in the same line.

To reproduce

Create multiple markdoc tags with different issues in the same line. There's no good way to figure out the start/end columns for each offending markdoc tag.

@mfix-stripe do you know what it would entail to fix this? we're running into a number of downstream issues in our app that are blocked on this. happy to take a stab at contributing if i could get a pointer
