Got the same issue here, with default French settings |
If you have your system set to 24-hour time (as OP does), I believe I've fixed this issue in my fork of the project. Release here: https://github.com/Ashe-Sterling/ElevenClock/releases/tag/1.5 |

I have a similar problem.
Primary monitor 21:01 Displayed on the second monitor %H:01
My region is Germany
Time short => HH:mm
Time long => HH:mm:ss
It would be great if you could fix the problem as well.
