This is two issues, not sure if related so will list them together (initially):
When triggering the extension the I am able to find the references and then the extension inserts the citation and focus stays with Zotero.
After inserting a number of citations, the citation picker popup allows the citations to be selected, however no citation is inserted into vscode, requiring a restart for vscode and the extension starts working again. This one is hard to replicate but has happened a number of times.
I have the beta version of Zotero installed as per the recommendation by @mblodehere.
