Rename "Subscription" to "Action Plan" and unify across platform

@grumo I think we can simplify our terminology by just using "Action Plan", for example "Add this to your Action Plan" or "Should I remove this from your Action plan". The idea is that every user has their unique Action Plan which is constantly synced with the live intent tree.

Does this make sense?

@shervine Yes. I like Action Plan much more than Subscription. Subscription has connotations we don't want like "payment subscription" or "newsletter subscription".
