Switch to Azure Account extension for authentication
- Current authentication is broken entirely in remote environments
Doesn't support modern express routing patterns (may not be possible to solve this)

Issues in remote environments

WSL paths aren't supported - fails to watch files in \\wsl$\Ubuntu-18.06\home\
- Prevents metrics details from launching from code lens
