Issue Type: Bug

Steps to Reproduce:

  1. Open terminal
  2. Write code which has available refactorings and is longer than editor viewport
  3. Select all
  4. Right click
  5. Click on "Refactor..." option

Does this issue occur when all extensions are disabled?: Yes

Expected result:
Refactor options should be displayed in editor viewport - e.g. in place where user did right click, not where selection ends up.

VS Code version: Code 1.31.0 (7c66f58, 2019-02-05T22:35:56.624Z)
OS version: Windows_NT x64 10.0.17134
