bug(ui): Manage subscription from setup screen.

If someone sets up a subscription with monetr and then deletes their last link, they can no longer access their subscription settings page to cancel or change it. They must add a new link (even a manual one) before they can.

The subscription management page should just be accessible from the setup page when a subscription is present.
