I'm wondering if we couldn't work around this problem by adding a couple of indices after the fact.

None yet. Right now I'm working around the same problem in a different application (Wallabag), which is what got me thinking about this. Give me a couple of hours to tinker over there, and then I'll examine the database on my own Monica install.
