Change <input type="password"> to be dynamically created

Following up on https://twitter.com/rlbarnes/status/689485125910855683

Our http://mozilla.github.io/pdf.js/web/viewer.html always shows page is unsecured warning due to our password field. We need to create this field dynamically to not raise this warning for all content/documents.
