Install/run an extension from a directory of source code

#6 builds an XPI then runs the extension. It can be modified to run from a directory of code (i.e. skip the build step).

This was done as part of #309 It doesn't need user documentation because there aren't any user controls for doing a proxy install. It is only done as a fallback for older Firefox versions.
