Not sure if I should make a preferred URL include the Month and Year as well as the Title. Does this improve SEO? Will this bring more people to the page because they know how "fresh" it is?

Yes, in the end I want to have the full blog entry url be
domain.com/blog/yyyy/mm/dd/title_of_entry
or domain.com/b/yyyy/mm/dd/title_of_entry
or just domain.com/yyyy/mm/dd/title_of_entry
