After creating a new diagram, filling it with valid text, saving it, closing it, and then reopening it from the preview sidebar, the diagram's image is blank. This happens because when a diagram is first created, Diagram.ImageFile does not exist, and so .Exists continues to return false. This file needs to be refreshed after saving,
