Don't default to showing name if logged in user has previously selected anonymous #150

Default to the previous setting for those with user accounts.

Comment via email 11/1/13:

Not sure if I messed up or there is a bug, but I posted my
first report anonymously, this created an account and then when
I posted my second report this shows my name publicly

User comment: " Having sent original posting anonymously, I did not realise that updates would not also, automatically be anonymous as the option boxes do not appear on the screen unless one scrolls down. Hence, my update, which was a clarification of location, appears with my name at the bottom."

User comment on 18/3/14: "It would be good if you could provide signed in users with the option to permanently set the option to not show their name"

"I have repeatedly unchecked "show my name publicly" but each time I log in, it is checked again. I understand you send my name to the council but do not want my name shown on the website."
