Description

The readme indicates that this module can be installed via npm i @newrelic/fastify, but that package doesn't actually seem to be published to npm.

Steps to Reproduce

npm i @newrelic/fastify

Expected Behavior

Should install from npm

this should be removed: https://www.npmjs.com/package/fastify-newrelic
