Search for free slots - practitioner reference #39

The practitioner reference is still coming through as an extension on the schedule resource when it should now be an actor. This needs changing in the backend and frontend of the demonstrator.

This has been completed in branch feature/slots-practitioner-reference

The back end now adds the practitioner as an actor on the schedule. All references to the practitioner have been altered to refer to an actor. Any assumptions of location being the only actor have been changed to check the reference.
