The following

module Make : functor (H : HashedType) -> S
                                          with type key = H.t
                                           and type 'a t = 'a Hashtbl.Make(H).t

should be indented

module Make : functor (H : HashedType) -> S
       with type key = H.t
        and type 'a t = 'a Hashtbl.Make(H).t

or even (better)

module Make : functor (H : HashedType) -> S
  with type key = H.t
   and type 'a t = 'a Hashtbl.Make(H).t

If the latter, the following current indentation

module Make (H : HashedType) : S
       with type key = H.t
        and type 'a t = 'a Hashtbl.Make(H).t

should also be made coherent.
