Allow user to create a new location during signin

During the Choose Location step on signin, a user with the appropriate permissions should be able to create a new location. This would actually be nice as we can delete all of the existing demo locations and require this as a first step in the on-boarding process.

I finally figured out how to pull down a PR as a branch

git fetch origin pull/1128/head:643-allow-user-to-create-location-on-signin
git checkout 643-allow-user-to-create-location-on-signin

I still need to figure out how to deal with the permission issue, but it works as I was expected. Well done!
