Division-Response_Area-FamilyNumber-ChildNumber

Division and response area come from the household_address for the family.

Family number is from the household.id.

Child number is the nth child of that particular household... not the child ID.

As per convo with @Jillzey we're dropping the compound family number in favor of a unique ID.

Thus... we can just use the id column.
