System crash when clicked Started action

Which feature of OpenCRVS your bug concern?

Record audit

To Reproduce
Steps to reproduce the behaviour:

  1. Login as FA/RA/ Registrar
  2. Go to In progress/ draft
  3. Go to record audit page
  4. Click on started action

Expected behaviour
Should open started modal

Actual behaviour
System crash

Screenshots

OpenCRVS Core Version:

V1.4.0, V1.3.3

Tested on:
Staging, QA
