Add a box on the results page for users to enter their email address for more information from San Mateo County HSA.

Ideally, each food program is associated with a mutliselect box and the site sends email to a specific email address associated with each program.

For example, a user who wants more info on CalFresh could enter their email address, check the CalFresh box and the site would email snap@hsa.smcgov.org

Still waiting for email addresses from the End Hunger Task Force. Will update this issue to higher priority once we receive the info
