Duplicate Question on IRS Field Office Form

• When the "Enter IRS Field Officer" form is opened the first question presents as a drop down. The drop down however opens in duplicate questions. See screenshot below.
  

• The question also does not provide data to select on the drop down for the name of the Field Officer. The client has asked that we provide dummy names (names previously used) until an updated list is provided.

• The question on the Health Facility catchment area is also in duplicate (See screenshot below)
  

• The drop down should provide the health facility catchment area assigned to the user.
