Each attachment upload should result in a newly created version of the note, in order to have a nicely browsable history.

This was pretty much fixed by allowing the user to choose when new versions are being created by simply pressing the "Save" button. Using a content check, Paperwork knows whether to create a new version or not. All attachments uploaded will then be attached to the latest version of the current note.
