Adjust the chainHead_follow events and errors with the changes introduced in:

Change the chainHead functions to have proper limits json-rpc-interface-spec#66
events have changed and are only reported on the chainHead_follow subscription stream
ensure they are synced with the spec in terms of serialized/deserialized format
add unit-tests for serialized/deserialized format
