Ball can clip into or through the flippers

In many cases the Continuous Collision Detection does not work well enough, and the ball will clip into the flippers. Or even go straight through it. This was also noted on Steam Community as feedback. There are repro's in videos there, but any considerable amount of real play will have you encounter this bug, though it can be hard to reliably debug.
