I have treestyletab set to open new blank tabs as the next sibling of the current tab. This works, i.e. if I press control+T or the new tab button.

But what if I want to open a bookmark in a new tab as a sibling? It always opens as the last tab, so I have to create the new tab first, then middle click the bookmark.

Middle clicking a link on a page opens nicely as the next sibling, so why not a bookmark?

I have no plan to add such an option, because I think TST should handle "tabs related to the current tab" but should ignore other tabs. I always recommend people to use other tab-management addon on like Tab Utilities.

Yes, actually there is an option for new blank tabs, but it is an exception - I added it because I really really annoyed from too many requests for the feature. (To be honest, I want to remove the option as soon.) It is my wish that no more option for various features not related to "tree of tabs".
